Travel Insurance Waiver Form!

By filling out this Waiver Form, you are confirming that we (Bill & Maria's - The Traveling Agent) Travel company have communicated and offered to you our RBC Travel Medical & Cancellation/Trip Interruption Insurance Coverage, and you have declined our travel insurance offer for your reasons indicated in this waiver form. Please fill out the form, and select your reason for declining. You can also place additional comments in the box if you wish:

IMPORTANT!

This is in relation to Group Bookings:

If you are part of a Wedding or Specialty group, and you're indicated on the group as the Group Coordinator, it is your full responsibility as the group coordinator to advise all your guests booked on your group of the importance of having some form of Travel Insurance Coverage while away from their home country. You must have some form of written proof that all guests were advised and aware of such coverage, and they have sufficient travel insurance coverage, or will be purchasing sufficient travel insurance coverage either from us or from another source, and if they're purchasing such coverage elsewhere will be declining our RBC Travel Insurance options by filling out this Waiver form below.

There are serious consequences as a group coordinator if proven that such important information was not communicated to your group guests, and you may be held liable for such damages that occur from any claim from a guest on your group that can prove they were not properly informed of this information.

If you have written proof from all your guests that they are sufficiently covered with qualified insurance coverage, or you have in writing from all guests that they will be declining our offered travel insurance, you may on your groups behalf fill out this waiver from by selecting "I Am Declining On Behalf Of Our Group" and place any additional notes in the message box if needed.
